Meet with Agate Healthcare Services to review your care inquiry, discuss the requested services, schedule, location, and caregiver preferences, and learn more about the consultation and referral process. Please complete the Caregiver Consultation Inquiry before scheduling this appointment. Consultations may last up to 60 minutes. Scheduling a consultation does not establish a care arrangement or guarantee caregiver availability.
Cancellation Policy
Consultation requests are subject to Agate’s approval. Please submit the Caregiver Consultation Inquiry before requesting an appointment. Consultations take place at the address entered during booking. Please contact Agate as soon as possible if you need to cancel or reschedule.
