Agate gathers the information needed to identify caregiver referral options while keeping you informed and involved in each decision.
1. Share Your Care Needs
Complete the care inquiry with details about the care location, support needed, preferred schedule, desired start date, and caregiver preferences.
2. Speak With Agate
During your consultation, we will clarify your priorities, answer questions, and discuss the caregiver qualifications that may be appropriate for your needs.
3. Review Referral Options
Agate reviews available independent caregivers in its registry and presents referral options whose qualifications and availability may align with your request.
4. Choose Your Caregiver
You decide whether a referred caregiver is the right fit. Once selected, Agate explains the remaining referral steps so you and the caregiver can establish the care arrangement.

Agate takes time to understand your care needs, reviews the qualifications and availability of independent caregivers registered with Agate, and presents referral options that may align with your request.

Agate remains your point of contact throughout the referral relationship. We coordinate administrative details, respond to concerns, assist when needs or availability change, and provide additional referral options when necessary.

Referred caregivers work as independent contractors and are responsible for the services they provide.

Once care begins, keep Agate informed of schedule changes, interruptions in service, concerns, or proposed changes to the care arrangement. Agate remains your point of contact for referral coordination and administrative support.
